Six in ten adults in the US live with a chronic disease and four in ten adults have two or more.  Chronic diseases like heart disease, cancer, stroke, and diabetes are the leading causes of death and disability in America. They are also leading drivers of the nation’s $3.3 trillion in annual health care costs.

Silicon Valley is home to a number of companies looking to improve how people manage their chronic conditions and to drive down the cost burden to society. But are these digital solutions really bending the trend? Let's talk to the companies creating the solutions and explore the impact they are having on care and costs.
